The concept of malignant narcissism was expanded by psychiatrist Otto Kernberg, who termed it a toxic combination of four highly dysfunctional traits and behaviors: Psychology Today:
Narcissism, with its grandiosity, lack of empathy, need for attention, and sense of entitlement.
Antisocial behavior, with its lack of remorse, destructive and impulsive behavior, deceitfulness, and disregard for and violation of the rights of others.
Paranoid thinking, with its sense of persecution, difficulty trusting others, preoccupation with others’ loyalty, tendency to bear grudges, and tendency to view benign actions of others as attempts at deception or exploitation.
Sadism, with its cruelty, efforts to humiliate or manipulate others, and deriving enjoyment from others’ pain and suffering.
5. Reckless disregard for safety of self or others

1. Failure to conform to social norms with respect to lawful behaviors as indicated by repeatedly performing acts that are grounds for arrest
2. Deceitfulness, as indicated by repeated lying, use of aliases, or conning others for personal profit or pleasure
3. Impulsivity or failure to plan ahead
4. Irritability and aggressiveness, as indicated by repeated physical fights or assaults
6. Consistent irresponsibility, as indicated by repeated failure to sustain consistent work behavior or honor financial obligations
7. Lack of remorse, as indicated by being indifferent to or rationalizing having hurt, mistreated, or stolen from another.
Examples of the cruelty: Ignoring due process to justify kidnapping and murder, cutting funding for programs for the poor and the disabled, forcing women to stay pregnant despite potential risk to their lives, inciting political violence, ruining people’s reputations by spreading lies about them, and putting immigrants and American citizens of color into dehumanising detention camps.
